COMMUNITY ONE-ON-ONE PROGRAM For those clients who live outside the Belden House in the Santa Rosa area, we offer one-on-one services by a trained, experiences caregiver. Hourly fees and compensation for expenses are determined based on the need level of the client, the actual expense of the activity, and mileage. The hours are set based upon the need, and may be altered with notice. Activities can be as varied and unique as the client that we serve, and can take place both at the client's home, and/or in the community. They may include: -Assistance with activities of daily living -Volunteering in the community -Sports and exercise -Shopping -Household chores and projects -Movies -Day trips to surrounding points of interest -Support while at work or school -Cooking -Visiting client's friends or relatives -Arts and crafts -Reading and music ONE-ON-ONE SERVICES FOR BELDEN RESIDENTS Occasionally one of our Belden residents may require one-on-one services. These may be provided by one of our experienced, trained staff at an additional cost, which will be determined based upon the need level of the client, and the expenses incurred (if the services require paying for outside activities, food and gas). The hours will be determined by the Belden House Administrators, and the resident's family or conservator, based on the need of the resident, and the wellbeing of the house. As with our Community One-On-One program, the daily activities will be based on client need and individuality, and may include: -Additional supervision at the Belden House -Supervision away from Belden House, which may include the type of activities listed above with our Community program. SATELLITE SUPPORT-HOMES (NON-LICENSED) Formative Care, Inc. owns two, two bedroom homes on either side of the Belden House, where we rent to high-functioning adults with brain injury. These homes are non-licensed support homes, focused on allowing the maximum amount of independence while still benefiting from a watchful presence next door. The homes are fully furnished. Residents supply linens, and personal touches.
